During our telephone evaluation, we will evaluate the care needs of your loved one, which may encompass standby assistance and support with daily living activities. Additionally, we will consider their cognitive capabilities and determine if they would benefit from an apartment-style environment with other active adults seeking social interaction, as well as services such as transportation, meals, and housekeeping. Alternatively, we will assess the suitability of memory care options that provide a safe environment for exploration.

 

The only prerequisite is the completion of the 602 Physician’s report by their primary care physician.
